Adam: I have had very good luck with Knology in HD + CableCARD + TiVo HD. Really, the only problems in my area have been some minor signal weakness issues due to the layout of my neighborhood [I live in Madison Town Centre, so it's a dense neighborhood to serve]. Knology was responsive to the Quality of Service issues, though, and I haven't had signal problems in months.

How is Knology's HD? Signal overall? With my TiVo and Cable Card, the majority of channels that would normally be digital are analog. I'm quite fed up with it . . .

I have seen the $1.99/mo quoted in other online discussions, but in conjunction with paying for digital + HD for both CableCARDs.

See, when I talked to someone at Knology, they told me that the CableCARD was only $1.99 a month (after I already had digital and HD). I do not remember they guy's name, but I did write down the case number so the next time I call back, I will definitely use that as a reference when they try to give me the $12.99 per card crap.
